Experience Survey

Introduction

The British Columbia College of Oral Health Professionals (BCCOHP) is conducting an anonymous patient/peer experience survey as part of its Quality Assurance Program. This survey is intended to better understand patient experiences, as well as the collaboration and contributions of oral health professionals, including those who do not provide direct patient care. Survey feedback is also intended to help oral health professionals reflect on their practice and identify opportunities for improvement.

This survey may be shared with you by an oral health professional or accessed through the BCCOHP website.

Your responses are anonymous and will only be shared in a way that cannot identify you. Responses are reviewed collectively by BCCOHP to support continuous improvements to the QAP and may be reviewed by your oral health professional to support reflection and learning, not evaluation.

Please do not include any personal or identifying information in your responses, including information that could identify yourself, the participating oral health professional or any other individual.

We encourage you to answer the questions honestly to support improvements in patient care and professional practice across the profession. Your participation is voluntary. You may choose to skip any question or stop the survey at any time.

Please note that this survey is not a mechanism for submitting a concern about your oral health professional. If you have concerns about the care you received, please submit them here.
